Feed Well is seeking a skilled and experienced International Board Certified Lactation Consultant (IBCLC) to join our multidisciplinary infant feeding team full-time. This role is ideal for a dedicated professional who values collaboration, continuous learning, and evidence-based care.

Required Qualifications:

• 3+ years in lactation or lactation adjacent field.
• Valid license (LPN, RN, SLP, PT, OT) and IBCLC certification, both in good standing.

Paid, Full-Time Hybrid Position

Feed Well Co. is accepting applications for a paid, full-time Pathway 3 IBCLC Clinical Internship. This 40-hour-per-week hybrid position combines hands-on clinical training, paid employment, directly supervised Pathway 3 experience, education, case review, documentation support, and individualized mentorship.

This is not an observation-only mentorship. It is an immersive clinical training and employment opportunity intended for healthcare professionals who want to become International Board Certified Lactation Consultants and build a long-term career with Feed Well.

Selected interns will participate directly in patient care within the boundaries of their existing professional license or credential, education, demonstrated competency, assigned supervision, Feed Well policies, and applicable IBLCE requirements. Clinical responsibility and independence will increase gradually as competency is demonstrated.

Feed Well is a multidisciplinary infant-feeding clinic dedicated to providing comprehensive, evidence-based care for infants and their families. Clinical Internship Experience

This is a hands-on training position. Interns will move from closely supervised participation toward greater clinical responsibility as knowledge, judgment, and competency are demonstrated.

Depending on the intern’s professional license, education, experience, demonstrated competency, patient availability, and required level of supervision, clinical opportunities may include:

• Participating in directly supervised lactation and infant-feeding consultations
• Developing comprehensive assessment, counseling, clinical-reasoning, and care-planning skills
• Supporting families with latch, positioning, breast or nipple pain, milk production, pumping, supplementation, and weaning
• Learning flange fitting, breast-pump selection, and individualized pumping-plan development
• Supporting bottle-feeding, bottle refusal, combination feeding, and formula-feeding families
• Assessing infant-feeding function within the intern’s professional scope
• Learning about oral function and appropriate pre- and post-frenectomy feeding support
• Supporting families experiencing plugged ducts, inflammatory breast conditions, or mastitis
• Discussing allergies, intolerances, reflux, poor weight gain, and other complex feeding concerns
• Supporting prenatal feeding preparation, return-to-work planning, and the introduction of complementary foods
• Reviewing routine and complex cases with experienced IBCLCs
• Developing clear, objective, accurate, and timely clinical documentation
• Creating realistic, evidence-based, family-centered management plans
• Learning when interdisciplinary collaboration, escalation, or referral is appropriate
• Coordinating care with IBCLCs, PTs, OTs, SLPs, nutrition professionals, pediatric providers, and other healthcare professionals
• Participating in structured virtual education, case discussions, documentation review, and individual mentor meetings
• Assuming progressively greater clinical responsibility as competencies are demonstrated

All clinical activities must remain within the intern’s existing professional license or credential, education, training, demonstrated competency, assigned level of supervision, Feed Well policies, and current IBLCE requirements.

Ideal Candidate

We are seeking applicants who:

• Intend to qualify for the IBCLC examination through Pathway 3
• Hold an applicable healthcare license or professional credential, such as RN, OT, or SLP
• Hold any required Maryland license or are eligible to obtain appropriate licensure before beginning clinical work
• Have completed relevant lactation-specific education and prerequisite training, such as CBS or CLC training
• Are eligible to work in the United States
• Are available to work a full-time, 40-hour-per-week hybrid schedule
• Can travel consistently to Feed Well’s Columbia and/or Glen Burnie locations
• Are interested in a long-term clinical role with Feed Well
• Demonstrate professionalism, reliability, initiative, sound judgment, and strong communication skills
• Are receptive to feedback and committed to continuous learning
• Value evidence-based care and thoughtful clinical reasoning
• Are committed to inclusive, anti-racist, and nonjudgmental support
• Respect all feeding methods and family-defined feeding goals
• Understand the importance of confidentiality, ethical practice, timely documentation, and professional boundaries

Pathway 3 Requirements

IBLCE currently requires Pathway 3 candidates to complete an approved Pathway 3 mentorship plan and at least 500 hours of directly supervised lactation-specific clinical practice obtained within the five years immediately preceding the examination application.

A Pathway 3 plan must be verified by IBLCE before clinical hours intended for Pathway 3 eligibility are earned. Actual participation in lactation care is required; observation or shadowing alone does not count as lactation-specific clinical experience.

Interns are responsible for reviewing and satisfying all current IBLCE eligibility, health-sciences education, lactation-specific education, clinical-experience, plan-verification, and examination-application requirements. Applicants should review the current IBLCE Pathway 3 requirements directly.

Not every hour worked will necessarily qualify as a Pathway 3 clinical hour. Countable hours will depend on the nature of the activity, the approved mentorship plan, required supervision, patient volume, mentor availability, scheduling, location, demonstrated competency, and current IBLCE requirements.

Participation does not guarantee that an intern will complete all required clinical hours by a particular date or become eligible for the IBCLC examination. Each intern remains responsible for tracking hours and confirming eligibility with IBLCE.

Compensation and Employment Commitment

This is a paid, full-time employment position, not a tuition-based or unpaid mentorship arrangement. Interns receive compensation for hours worked while participating in supervised clinical training, education, case review, documentation support, and mentorship. Specific compensation and applicable employment benefits will be discussed during the interview and offer process.

Feed Well makes a substantial investment in each intern’s education, supervision, mentorship, and professional development. Accordingly, this internship is intended to create a long-term employment pathway with Feed Well, rather than serve solely as a means of obtaining Pathway 3 clinical hours.

Selected candidates will be expected to enter into a separate written agreement outlining a two- to three-year post-mentorship employment commitment. The agreement will describe the applicable expectations, duration, and terms and will be provided separately for the candidate’s review before acceptance.

Continued employment and progression within the program will remain subject to satisfactory performance, professional conduct, continued licensure or credential eligibility, successful completion of required competencies, compliance with Feed Well policies, and the terms of the applicable employment and internship agreements.

This is a full-time, 40-hour-per-week hybrid position that includes both in-person and virtual responsibilities.

• Virtual care: Used for appropriate patient-care experiences, education, case review, documentation support, and mentor meetings

Interns must be able to attend required in-person clinical experiences. The specific distribution of hours and locations will depend on clinic operations, mentor scheduling, patient volume, supervision requirements, and the intern’s training needs.
